With your Delta Dental PPO (Point-of-Service) plan you can visit any licensed dentist. You will usually save the most money by choosing a participating Delta Dental PPO dentist. Use our online dentist search to find out if your current dentist is part of the Delta Dental network.

What’s the Member Portal? 

Member Portal gives you easy, secure access to your benefit information 24 hours a day, seven days a week. Review eligibility, annual maximum used to date, levels of coverage, and claims information. You can also search for a dentist, print your your member ID card, order replacement ID cards and sign up for electronic EOB statements using this innovative tool.
To access the Toolkit, you will need to register first. To register you will need the following:

• Subscriber's first and last name

• Subscriber's date of birth

• Member ID (use Social Security number)

• Security question

Print your own member ID card:

To print an ID card, you will need to log into Member Portal. Once logged in, click the Print ID Card link on the left-hand menu. You will see your member ID card displayed on the screen. Use your browser's print function to print as many copies as desired. You now have the option to order replacement ID cards as well. 

Need a claim form?

Download a claim form HERE. Remember, when you visit a participating Delta Dental dentist, the dentist will complete and file claim forms for you. If you choose to visit a dentist who does not participate with Delta Dental, you may need to file your own claim forms.
